Providing low-barrier access to services
After another successful season of the Code Blue Program at CERAH and a busy summer of Supportive Services, we are proud of the work we have been able to accomplish, ensuring low-barrier access to critical human services to Phoenixville area individuals and families experiencing housing insecurity.

Moving people forward
While we may not be able to provide a solution for everyone we serve, we are proud of the incredible partnerships we have built to ensure that the majority of our guests have a long-term plan.

86% of those we served were placed in permanent housing, long-term shelter programs, in-patient drug & alcohol treatment programs, and/or supportive housing. Because of the trust built through supportive services and strong ties to community partners, we can move more people up and out of the cycle of homelessness.
GUEST STORY
Katharine* had been using the Code Blue Program for winter services off and on for several years. At 35 years old, Katharine has spent nearly one third of her life, living mostly outdoors, as a person experiencing homelessness. Because of her situation, including mental health issues she refused to address due to pride as well as untreated medical issues, Katharine struggled to trust individuals, programs, and systems. Because our Code Blue Program did not have many of the barriers individuals sometimes experience, she felt comfortable here. This allowed us to develop a trusting relationship and help her access additional services.
Today, Katharine is working closely with our staff to accomplish goals she never saw herself being able to achieve. Through a partnership with Phoenixville Hospital, we were able to gently guide her towards getting a mental health diagnosis and treatment. We've assisted her with acquiring health insurance, finding doctors, and helping her schedule appointments to treat her medical issues. Now enrolled in our GED program, she is gaining the tools she needs to manage her learning disabilities. She's working hard to learn to read and spell so she can obtain employment which, up to this point, has had very limited options for her. We've also worked with a partner non-profit which has offered her housing when she is comfortable and ready to move forward.

All year at the HEART Kitchen
Across our four main food programs, the HEART Kitchen averages 500 meals per week. Every meal program is FREE to the community we serve and supported by talented volunteers who give their time to ensure everyone has a seat at the table.

Fesh meals warm hearts
We cannot stress enough what a difference a hot, fresh meal makes for those we serve. Our guests at CERAH are offered amazing meals created by the staff and volunteers of the HEART Kitchen every evening and morning.

For some, it has been the comfort of childhood favorites or access to culturally appropriate foods that are too expensive or hard to come by when you are exeriencing homelessness.

CODE BLUE
Summer Meal Program Returns
Once again we partnered with the Phoenixville Area School District (PASD) and Phoenixville Area Community Services (PACS) to provide the Summer Meal Program to PASD families who qualify for free or subsidized lunch throughout the school year.

Over the course of the 11 week summer break, Ann's Heart served 16,320 meals - including breakfast, snacks, and dinner.

Education Empowers
With the launch of the Adult GED Program in 2022, our Volunteer Educators opened the door for local adults in search of improving their lives through education to complete their GED.

Thanks to the generosity of the Justamere Foundation, our program now provides free one-on-one tutoring, supplies, and testing for adults continuing their education. This year we have already celebrated 5 graduates and have several more just a few tests away from completing their GED!

The Board of Directors and Staff at Ann's Heart are committed and honored to serve our community. We are grateful for the recognition our programs and staff have received over the last fiscal year.

CERAH Program Manager, Karin Williams, was recognized as Phoenixville Community Health Foundation's Champion of the Community. In addition, our Executive Director, Development Director, and HEART Program Manager were nominated as well.

And in March of 2023, the Phoenixville Regional Chamber of Commerce awarded Ann's Heart the honor of "Non-Profit of the Year"!
