Housing Case Management Request Form

Housing Case Management is available to qualifying individuals from the Phoenixville area who are homeless or at imminent risk, AND who have called 2-1-1 to be connected to Chester County's Coordinated Entry System. Anyone interested in case management must complete our intake process and provide proof of residency.

We do NOT provide cash assistance, gift cards, rental or utility assistance.​
